The Conway Area Humane Society reserves the right to close the Conway Area Humane Society Dog Park at any time for maintenance purposes and/or repairs.
The Conway Area Humane Society Dog Park is not supervised by any agent or employee of CAHS. Use of this facility is at your own risk. By using this facility, you agree that you understand and assume all responsibility and liability for the risks of unleashing your dog(s) in this area.
I, the undersigned, have read and understand the dog park etiquette guide. I understand that if the etiquette guide is not followed it may result in restricted use of the park, dog training, and termination of membership. I further understand that it is my duty to report incidents in the dog park.
Release and Hold Harmless Agreement
Now comes the undersigned and in consideration of access to and/or membership in the Conway Area Humane Society Dog Park (hereinafter “the Park”) agrees to release and hold harmless The Conway Area Humane Society (hereinafter “CAHS”) and all of its employees, volunteers, staff, personnel, officers, and directors from any claims of any nature whatsoever which may arise or flow from use of the Conway Area Humane Society Dog Park or from presence of the undersigned upon any portion of the CAHS property, including parking areas. Such claims include, but are not limited to, any injury, illness, disease, property damage, medical, or veterinary expense. In addition, the undersigned agrees to indemnify and hold harmless CAHS and all of its’ employees, volunteers, staff, personnel, officers, and directors from any claims which may be made against them arising from the use by the undersigned or any person or animal brought onto the premises by the undersigned, of the Conway Area Humane Society Dog Park and CAHS. In addition, the undersigned hereby agrees to be personally responsible for any damage of any type caused to the Park or any portion of the CAHS property by use of the undersigned or any other person or animal brought upon the property by the undersigned.
THIS AGREEMENT IS LEGALLY BINDING. OBTAIN ADVICE BEFORE SIGNING IF NECESSARY.
